About The Position

This role is for an LPN Clinic Nurse at LCMC Health, specifically within a Wound Care Clinic. The LPN will deliver patient care under supervision and in collaboration with the healthcare team, measure and record vital signs, support clinic workflow, prepare patients for examinations, identify and report signs of abuse or neglect, recognize and respond to emergencies, perform non-complex clinical procedures, ensure patient comfort, and prepare, clean, and disinfect clinical equipment. Additionally, the LPN will administer prescribed medications and vaccinations, collect and test specimens, and maintain medication inventory. Documentation responsibilities include accurately recording clinical tasks, procedures, and test results in the patient's medical record, completing quality control records, and maintaining patient health information confidentiality. The LPN will utilize the Electronic Medical Record (EMR) system for various tasks, including accessing results, updating information, scheduling appointments, and submitting safety reports. Professionalism, communication with the clinic manager, and adherence to safety, environmental, infection control, and regulatory standards are also key aspects of the role. The LPN will participate in quality improvement initiatives and provide clerical support, including operating office equipment, scanning documentation, obtaining medical records from outside facilities, and handling telephone calls.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the state of Louisiana
  • Current Basic Life Support (BLS) Healthcare Provider Certification from the American Heart Association

Responsibilities

  • Deliver patient care under supervision and in collaboration with the healthcare team.
  • Measure and record vital signs in accordance with departmental policy and patient age; recognize and report abnormal findings appropriately.
  • Support clinic workflow by assisting with patient intake and rooming, clinical procedures, scheduling new and follow-up appointments, and coordinating patient flow and discharge.
  • Prepare patients for examinations, treatments, and procedures.
  • Identify and report signs or symptoms of abuse or neglect in accordance with policy.
  • Recognize emergencies and respond appropriately following organizational protocols.
  • Perform non-complex clinical procedures (e.g., simple dressing changes) within scope of practice and documented competency.
  • Ensure patient comfort with consideration for age, condition, and special needs.
  • Prepare, inspect, clean, and disinfect clinical equipment in accordance with hospital standards.
  • Administer prescribed medications and vaccinations per physician orders and departmental guidelines.
  • Collect, prepare, and secure specimens for point-of-care testing or transport; perform testing per laboratory guidelines.
  • Maintain medication inventory, including proper storage and labeling, safe handling of multi-dose vials, disposal of expired medications, and monitoring of refrigeration/freezer logs for compliance.
  • Accurately document all clinical tasks, procedures, and test results in the patient’s medical record in accordance with established guidelines.
  • Complete required documentation, including quality control records for waived testing.
  • Maintain the confidentiality and security of patient health information at all times.
  • Utilize the Electronic Medical Record (EMR) system to access laboratory and radiology results, update patient demographic information, retrieve medical records, schedule follow-up appointments, and prepare and submit safety reports when required or directed by leadership.
  • Communicate effectively with the clinic manager regarding operations, issues, and resource needs.
  • Maintain professionalism and strict patient confidentiality at all times.
  • Support compliance with safety, environmental, infection control, and regulatory standards.
  • Implement standard and transmission-based precautions as required.
  • Promote and maintain patient safety throughout clinic visits.
  • Ensure a clean, organized, and safe work environment.
  • Report safety hazards, incidents, or policy violations promptly.
  • Participate in quality improvement initiatives, including QAPI (Quality Assurance and Performance Improvement) activities and Environment of Care (EOC) rounds.
  • Assist with clinic preparation by gathering relevant patient documentation for provider review.
  • Operate office equipment, including computers, printers, scanners, and fax machines.
  • Scan and upload external documentation into the EMR accurately.
  • Obtain medical records from outside facilities as requested.
  • Answer and route telephone calls in a timely and professional manner.
