About The Position
Accurate Home Care and its Home Health service line is growing and adding an LPN Visit Nurse to its team. At Accurate Home Care, our Home Health team aids in empowering our patients to stay at home and recover. This strong team of home health professionals manages the care of our patients while building relationships and ensuring a positive patient experience. The LPN Visit Nurse plays a key role in direct nursing care in the patient home.
Requirements
- Must be licensed as a Licensed Practical Nurse (LPN) in Minnesota
- Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies
- Must be able to lift, turn, position, and transfer up to 50lbs
- Must be fluent in the English language
- Valid U.S. driver’s license with minimal moving violations and access to a reliable personal vehicle
Responsibilities
- Complete patient visits as needed under the direction of the RN Case Manager.
- Participates in the implementation and oversight of nursing and home health aide services.
- Nursing skills may include phlebotomy, wound care, catheter care, medication management and patient education.
