Responsibilities

Provide hands-on nursing care guided by individualized care plans, ensuring each client receives the support they need to remain safe and independent at home.
Monitor changes in health and well-being, promptly reporting updates to the care team and ensuring continuity and quality of care.
Document your visits in the electronic medical record (EMR) with accuracy and clarity, helping the entire team stay connected and informed.
Participate in on-call rotations, including evenings, weekends, and holidays—providing urgent support with calm professionalism when it’s needed most.
Join nursing case conferences, attend team meetings, and complete ongoing training, contributing to a collaborative environment that strengthens care quality and professional growth.
