About The Position

Concordia is hiring a full-time LPN Unit Clerk to help keep resident care running smoothly while supporting a dedicated nursing team. As an LPN Unit Clerk, you’ll be a key part of the unit—administering medications, coordinating physician orders, communicating with families, and helping ensure residents receive the compassionate, high-quality care they deserve.

Requirements

• Current Pennsylvania licensure as a Practical Nurse
• Dedication to providing high-quality care
• Caring and compassionate approach to resident care
• Excellent written and verbal communication skills
• Ability to work well with residents, families, coworkers, and providers
• Demonstrated leadership ability

Responsibilities

• Administer medications, treatments, and procedures while following all care standards.
• Complete resident assessments and support care planning, follow-up, and documentation.
• Communicate resident updates with physicians, residents, families, and the care team.
• Process physician orders, referrals, paperwork, and daily reports.
• Help supervise unit care, support Nurse Aides, and promote quality outcomes.
• Build positive relationships and help resolve concerns professionally.
