About The Position

Buena Vida Rehabilitation and Nursing Center is seeking an LPN Supervisor to oversee and coordinate nursing care activities during assigned shifts. This role ensures safe, high-quality, and compliant resident/patient care. Working under the Director of Nursing or Registered Nurse, the LPN Supervisor provides leadership, guidance, and supervision to LPNs, CNAs, and other direct care staff, while also delivering hands-on nursing care within the scope of the Nurse Practice Act.

Requirements

  • LPN (Licensed Practical Nurse) licensed by the State NYC
  • 1-2 years of Long Term Care (LTC) required
  • Must have a reliable source of transportation
  • Must have a valid driver's license and automobile insurance

Responsibilities

  • Provide direct nursing care in accordance with physician orders, care plans, and facility policies.
  • Administer medications, treatments, and procedures within LPN scope of practice.
  • Monitor resident condition and promptly report changes to appropriate personnel.
  • Assist in implementing and updating individualized care plans.
  • Ensure admission and routine patient/resident observations/transfer notes are complete and accurately reflect the patient's/resident's status.
  • Report changes in a patient's/resident's physical/psychological condition appropriately.
  • Maintain knowledge of equipment set-up, maintenance, and use (e.g., monitors, infusion devices, drain devices).
  • Maintain confidentiality and patient/resident rights regarding all patient/resident/personnel information.
  • Provide patient/resident/family/caregiver education as directed.
  • Initiate emergency support measures (e.g., CPR, protects patients/residents from injury).
  • Conduct patient and family teaching according to the nursing care plan.
  • Ensure explanations are given to the patient prior to intervention.
  • Complete and accurate discharge/death summaries.
  • Complete and accurate transfer forms.
  • Utilize Facility Policy and Procedure Manual or reference materials as needed.
  • Perform procedures according to method outlined in procedure manual.
  • Follow safety guidelines established by the facility (e.g., proper needle disposal).
  • Ensure the patient's/resident's full name and room number are present on all chart forms.
  • Ensure allergies are noted on the chart cover.
  • Complete unit flow sheets properly (e.g., wound care records, treatment records, weight sheets).
  • Chart medications correctly with name, dose, route, site, time, and initials of nurse administering.
  • Write appropriate notes for medications not given and actions taken.
  • Follow the procedure for administration and counting of narcotics.
  • Chart all parenteral fluids, including additives, with time and date started, time infusion completed, site of infusion, and signature of nurse.
  • Schedule tests and complete preps as indicated.
  • Inform co-workers of changes in patient conditions or any other changes occurring on the unit.
  • Relay information to other members of the health care team (e.g., physicians, respiratory therapy, physical therapy, social services) and family/responsible party.
  • Coordinate unit activities (e.g., changing patients/residents’ rooms for admissions, coordinating transfer/discharge forms).
  • Supervise LPNs, CNAs, and unlicensed personnel during assigned shifts.
  • Make equitable care assignments that are appropriate to patient needs prior to the beginning of the shift.
  • Communicate staffing needs to the nursing supervisor.
  • Provide assistance, direction, and education to unit personnel and families.
  • Identify problems, gather data, suggest solutions, and communicate appropriately.
  • Assign duties based on resident acuity and staff competency.
  • Provide coaching, guidance, and support to nursing staff.
  • Monitor staff performance and policy compliance.
