Requirements

  • Valid, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state
  • Strong clinical assessment and critical thinking skills
  • CPR and other relevant certifications as required
  • Knowledge of geriatric nursing principles

Nice To Haves

  • Three (3) years of experience in senior living nursing
  • Prior supervisory experience

Responsibilities

  • Provides leadership and direction to the nursing team, including Registered Nurses (RNs), Licensed practical Nurses (LPNs), Qualified Medication Assistants (QMA’s)and Certified Nursing Assistants (CNAs) or State Tested Nurse Assistants (STNAs), ensuring the delivery of high-quality, resident-centered care.
  • Performs supervisory functions: Participates in the hiring, onboarding, and orientation process for new employees. Promptly and thoroughly addresses employee disciplinary action and terminations in coordination with human resources. Provides training, coaching and supervision to employes. Provides timely and accurate performance management evaluations and participates in goal setting for the employee, in accordance with the organization’s established policies and procedures. Prepares, implements, and monitors staffing schedules and work assignments. Administers department and organizational policies and procedures in a fair and consistent manner. Completed the payroll process in a timely and accurate manner.
  • Provides comprehensive nursing care to residents in accordance with their individual care plans, ensuring their physical, emotional, and psychosocial needs are met.
  • Conducts initial and ongoing assessments of residents’ health status, documenting observations, and communicating findings to the healthcare team.
  • Administers medications and treatments as prescribed and within scope of practice, following established protocols and ensuring accuracy and safety.
  • Monitors residents’ vital signs and response to treatments, promptly reports any changes or concerns to the physician and/or director of nursing.
  • Collaborates with physicians, nurse practitioners, and other healthcare providers to develop and implement effective care plans tailored to the residents’ unique needs and preferences.
  • Coordinates and oversees resident care and services, including the delivery of care and services by other qualified health care staff, life enrichment functions, meal service, housekeeping services and all other aspects of care and service within the nursing department.
  • Provides health education and support to residents and their families, promoting healthy lifestyle choices and facilitating access to resources and supporting services.
  • Fosters a positive and supportive environment for residents, promoting independence, dignity, and respect while ensuring their safety and well-being and in adherence with Resident’s Rights.
  • Maintains accurate and current medical records and documentation, adhering to state and federal regulatory standards.
  • Adheres to ethical and professional standards of nursing practice, demonstrating integrity, empathy, and commitment to excellence in care delivery.
  • Serves as a liaison between nursing staff, residents, families, and other healthcare professionals, facilitating communication, resolving concerns, and promoting positive relationships.
  • Conducts and participates in staff meetings, committees, and organizational initiatives representing the nursing department and contributing to decision making and budgetary and other planning efforts.
  • Performs other duties as assigned.
