About The Position

Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse. This role involves providing health education, routine LPN nursing care, observing and reporting patient reactions and conditions, and documenting services rendered and communications. The job description reflects management’s assignment of essential functions and may be revised as needed.

Requirements

• Associate’s degree required.
• Diploma as a Licensed Practical Nurse from an accredited program.
• Current, valid LPN license issued from NYS Department of Education.

Nice To Haves

• Experience in home health care preferred.
• Community health experience preferred.

Responsibilities

• Provide health education according to the teaching plan established by the RN.
• Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
• Observe signs and symptoms, and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager.
• Document any services rendered to the patient, changes noted in the patient’s condition, and/or family and home situations.
• Document communication with RN Case Manager and physician.
• Other duties as assigned.
