About The Position

The purpose of this position is to serve as a member of a multi-disciplinary care team to facilitate quality care, guided by guests’ individualized care plans, Facility policies and procedures, and State and Federal guidelines.

Requirements

  • Graduate of an accredited school of nursing.
  • One (1) year experience as a nurse.
  • Be familiar with or be willing to learn about State and Federal guidelines pertaining to long-term care.
  • Maintain a current comprehensive knowledge of nursing theory and practice.
  • Current licensure as an LPN or RN in the State of Montana
  • Current CPR certification; up-to-date knowledge of First Aid
  • Completion of 24 continuing education credits in 2-year period of licensure, as required by the State of Montana
  • The incumbent must be at least 18 years of age, as position operates potentially hazardous equipment and handles and dispenses prescribed medication.
  • The incumbent must be able to perform multiple tasks simultaneously and work under time and/or deadline pressures.
  • Required vision abilities include close up and distance, peripheral vision, depth perception, and ability to adjust focus.

  • Job duties typically involve frequently changing conditions and problems, requiring considerable judgment in applying factual background and fundamental principles to the development of approaches and techniques for the solution of problems.

Responsibilities

  • Follow physicians’ orders, treatments, and guest care plan approaches.
  • Carry out duties assigned by Charge Nurse.
  • Accurately assess guests’ conditions, document the same in guests’ medical record (eg. treatment, vital signs, I & O, behaviors), and communicate changes and other pertinent information to physicians, guest family members or designee(s), and members of the team when necessary.
  • Administer medications and treatments.
  • Complete medical and behavioral observations as required.
  • Work and coordinate with all team members to meet resident’s needs.
  • Assist nursing and other staff assisting guests, as needed.
  • Perform Universal Worker tasks, such as laundry, housekeeping, meal duties, maintenance tasks, and resident recreation if needed.
  • Work independently, while prioritizing duties and responsibilities, taking on additional assignments, maintaining accuracy and timeliness with documentation, and demonstrating the ability to problem solve.
  • Respond effectively to sensitive inquiries or complaints from others.
  • Serve as a role model for the Immanuel Spirit.
  • Supervise and administer nursing care as assigned by the Charge Nurse.
  • Ensure that Nursing team members are completing duties.
  • Supervise employee job performance by encouraging, educating, praising, and holding staff accountable.
  • Complete required documentation, including resident evaluations, Medicare documentation, care plan entries, daily progress notes, and weekly summaries before the end of shift.
  • Update resident care plans and transfer information onto safety sheets on ambulation equipment, beds, and other staff worksheets, CNA assignment sheets.
  • Accurately document medications administered, care plan approaches tried before PRN medication administration, and need for PRN medication, as well as follow up with efficacy.
  • Order and charge out medications and supplies, as needed.
  • Take physicians’ orders and transcribe them onto appropriate forms according to standard practice (eg. med / treatment sheets, lab slips).
  • Effectively work and communicate with a wide range of individuals from all socio-economic groups, including geriatric residents and those with special needs.
  • Communicate on a regular basis with guests/ residents and/or their designees regarding medical care issues and interventions.
  • Communicate verbally and in writing to other staff members regarding resident care issues.
  • Communicate with the charge nurse on a routine basis.
  • Ensure that information is communicated to team members so that they have all the information necessary to provide excellent customer service.
  • Communicate with physicians and other health care professionals as required .and follow up with pharmacist recommendations.
  • Participate in Nursing and staff meetings, as required, and training and/or meetings that help facilitate the departmental communication process.
  • Attend workshops and training sessions related to nursing practice, geriatrics, long-term care, etc., as assigned.
