About The Position
Phoenix Home Care & Hospice is seeking LPN/RNs for private duty care. This role offers a change from facility chaos and overloaded assignments, providing consistent schedules with long-term cases. Nurses will work close to home, building meaningful relationships with patients and families, and becoming part of their support system. The position focuses on providing skilled, one-on-one nursing care in the home, with an emphasis on compassion, integrity, and unwavering support. It is suitable for nurses seeking meaningful patient relationships, those burned out from facility work, semi-retired nurses, and clinicians who want to serve their local community.
Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
