About The Position

Angels of Care Pediatric Home Health is seeking compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile children and young adults in their homes. This role involves focusing on the medical needs and treatment of infants, children, and/or adolescents according to their active plan of care. The position offers full-time and part-time shifts, including days, nights, and weekends, with flexible scheduling options for 8, 10, 12, and 16-hour shifts. Angels of Care has been serving the medically fragile community nationwide since 2000, guided by values of Heart, Advocacy, Love, Outreach, and Speed.

Requirements

Active RN or LPN/LVN license
Provide care in a client home setting

Responsibilities

Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
Conduct on-going patient care and assessments.
Administration of prescribed medication, treatments, and therapies.
Coordination of care
Educate family members on patient clinical care to enhance positive outcomes
Preventative initiatives to protect quality of care for patient
