LPN / RN Pediatric Home Health Nurse
About The Position
Angels of Care Pediatric Home Health is looking for Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in the home setting. The role involves focusing on the medical needs and treatment of infants, children, and/or adolescents according to the active plan of care. This includes conducting ongoing patient care and assessments, administering prescribed medication, treatments, and therapies, coordinating care, educating family members, and implementing preventative initiatives. Requirements
- Active RN or LPN/LVN license
- Provide care in a client home setting

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
