LPN / RN Pediatric Home Health Nurse
About The Position
Angels of Care Pediatric Home Health is seeking compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role offers the opportunity to make a significant difference in the lives of patients and their families. The position involves ongoing patient care, assessments, medication administration, care coordination, and family education, all while adhering to a physician's plan of care. Angels of Care is committed to a patient-centered approach and fostering a supportive company culture.
Requirements
- Active RN or LPN/LVN license
- Provide care in a client home setting
- Ability to make a positive and lasting impression!

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
