LPN/RN Pediatric Home Care - Day Weekend schedule
About The Position
At Phoenix Home Care & Hospice, we create New Beginnings for both our patients and our nurses by leading with compassion, integrity, and unwavering support. This opportunity allows nurses to provide skilled, one-on-one nursing care in the home, administering and managing medications, monitoring and documenting patient conditions, performing treatments as ordered by the physician, and educating and supporting family members. Nurses will collaborate closely with the Phoenix clinical team. This role is ideal for nurses burned out from facility work, those seeking meaningful patient relationships, semi-retired nurses, and clinicians who want to serve their local community.
Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver's license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
