You will administer and manage medications, monitor and document patient conditions, perform treatments as ordered by the physician, and educate, encourage, and support family members. You will also work alongside the Phoenix clinical team to provide exceptional care. Requirements

• Active Illinois RN or LPN license (or Compact license)
• Strong assessment and communication skills
• A compassionate heart, dependable character, and self-motivated spirit
• Valid driver's license and reliable transportation

Responsibilities

• Provide compassionate, one-on-one nursing care in the comfort of your patients' homes.
