About The Position

At Phoenix Home Care & Hospice, we create New Beginnings for both our patients and our nurses by leading with compassion, integrity, and unwavering support. This role offers the opportunity to provide skilled, one-on-one nursing care in the home, focusing on pediatric patients. You will administer and manage medications, monitor and document patient conditions, perform treatments as ordered by the physician, and educate and support family members. This position is ideal for nurses seeking meaningful patient relationships and who are looking for an alternative to facility-based chaos. We offer consistent schedules with long-term cases, allowing you to work close to home and become part of a patient's support system during critical moments.

Requirements

• Active IL RN or LPN license (or Compact license)
• Strong assessment and communication skills
• Compassionate, dependable, and self-motivated
• Valid driver's license and reliable transportation
