About The Position
Phoenix Home Care & Hospice is seeking LPN/RNs for pediatric home care. This role offers a change from facility chaos and overloaded assignments, providing consistent schedules with long-term cases. Nurses will work close to home, building meaningful one-on-one relationships with patients and families, and becoming part of their support system. Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
- A great fit for nurses burned out from facility work, those seeking meaningful patient relationships, semi-retired nurses, and clinicians who want to serve their local community
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
