About The Position

Willow Valley Communities is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) for their new Marlin and Doris Thomas Memory Center in Willow Street, PA. This role is part of a founding team for an innovative household model of dementia care that focuses on empowering Residents with purpose, autonomy, and dignity. The center aims to change the perception and experience of dementia care by focusing on strengths, possibilities, and whole-person wellness. The Household Care Partner will be instrumental in this groundbreaking approach.

Requirements

• Active Pennsylvania Licensed Practical Nurse (LPN) License
• Experience with Memory Care preferred
• Passion for providing exceptional Resident-centered care
• Strong communication and teamwork skills
• Commitment to Willow Valley Communities' culture of excellence and compassion

Responsibilities

• Leading and coordinating nursing care on your assigned shift
• Administering medications and treatments according to physician orders and nursing standards
• Monitoring Resident health and responding appropriately to changes in condition
• Assisting with the development and updating of Resident Care Plans
• Responding to emergency and life-saving situations following established protocols
• Collecting and processing specimens for testing
• Supporting Residents with activities of daily living as needed
• Monitoring nutrition, hydration, and meal-time experiences
• Responding promptly and compassionately to Resident needs
• Collaborating with interdisciplinary team members to provide exceptional care
