About The Position
Phoenix Home Care & Hospice is seeking a Licensed Practical Nurse (LPN) for Private Duty Nursing (PDN). This role offers a chance to escape the fast-paced hospital environment and provide meaningful, one-on-one care to children and young adults in a home setting. The position emphasizes personalized care, building strong relationships with patients and families, and offers structured shift options including Full-Time, Part-Time, or PRN. Requirements
- Active LPN license (state required)
- Dependable, compassionate, and self-motivated
- Comfortable working independently in a home setting
- Strong communication and observational skills
Responsibilities
- Provide individualized, in-home nursing care
- Administer medications and treatments as prescribed
- Monitor patient condition and respond to changes
- Assist with daily living needs in accordance with the care plan
- Build strong, trusting relationships with patients and families
