LPN Resident Service Manager | Senior Living Community

Clermont ParkDenver, CO
Onsite

About The Position

This role supports residents living independently who need some additional coordination of care and services. It's more of an administrative nursing position, but one built on frequent resident contact — giving you the opportunity to develop strong, long-term relationships with the people you serve. The Resident Services Coordinator promotes a healthy lifestyle and manages the resident care experience through clear communication between the community's on-site Medical Clinic, therapies, ancillary services, and the interdisciplinary team. This role develops partnerships with outside service providers and agencies for resident referrals, educates residents on available services, and monitors the provision of those services. The Coordinator fosters a continuum of care for residents and serves as a liaison between families, residents, and healthcare professionals as needed.
