The LPN on the Ready for Discharge Unit will provide direct patient care under the direction of an RN's plan of care. This includes performing focused assessments, developing and implementing focused plans of care, and evaluating patient responses and progress. The LPN will also be responsible for accurate documentation in the electronic health record, assisting with patient and family education regarding discharge plans and medication instructions, and collaborating with the interdisciplinary team to ensure care continuity and smooth transitions. Additionally, the LPN will adhere to hospital policies, report concerns, demonstrate cost containment, and participate in quality improvement initiatives to enhance patient outcomes and discharge efficiency.
