About The Position

This is a full-time LPN position in an outpatient Internal Medicine practice providing services for adults 18 years and older. The role includes preventative health maintenance, and managing chronic and acute care needs. The LPN will be part of a team that includes Ambulatory Techs, RNs, Physicians, and APP's. Responsibilities include direct patient care, administering medication/vaccines, performing minor laboratory testing, and communicating with patients and providers using EMR. The practice is committed to fostering an inclusive and welcoming culture.

Requirements

  • At least 6 months in direct nursing care preferred but not required.

Responsibilities

  • Provision of direct care to assigned patients in an outpatient setting.
  • Contribution to unit activities and support of the care delivery system.
  • Assisting other staff members with implementing patient care.
  • Assuming responsibility of care for assigned patients under the direction of the co-assigned RN or MD.
  • Participating in the plan of care.
  • Implementing appropriate nursing interventions.
  • Contributing to patient/family education through data collection, interdisciplinary team collaboration, documentation, and communication.
  • Direct patient care.
  • Administration of medication/vaccines.
  • Minor laboratory testing.
  • Communication with patients and providers using EMR.
