About The Position

This role involves utilizing the nursing process for administering care to both well patients and those with health problems in an outpatient setting. The LPN will function as a member of the healthcare team, contributing to a safe and pleasant patient care environment. Responsibilities include documentation, patient preparation for procedures, data collection, assisting healthcare providers, and maintaining knowledge of patient population needs.

Requirements

  • Graduation from a Licensed Practical Nursing Program
  • Maintain competency in professional nursing practices
  • Current license and registration to practice Practical Nursing in New York State
  • Assumes responsibility for maintaining current knowledge related to the age and development of the patient population in areas in which they work
  • Current license and registration to practice Nursing in New York State
  • Infection Control
  • BLS Certification
  • Ability to complete basic mathematical calculations
  • Ability to complete tasks in efficient/timely manner
  • Ability to provide required nursing care under the direction of the medical provider
  • Ability to communicate effectively, verbally and in writing with personnel, patients, and medical providers
  • Ability to work effectively, in both cooperative and independent situations under the directions of the medical provider
  • Ability to recognize patient care problems and to react appropriately in patient care emergencies
  • Ability to seek and utilize nursing resources
  • Responsibility for nursing self-development and keeping current with advances in nursing knowledge and techniques

Responsibilities

  • Documentation of all patient care is required
  • Prepare patient by collecting and sharing data; blood pressure, pulse, respiratory rate, height/weight, Date of LMP, general history and chief complaint and identifies need for medication refills.
  • Collection of JCAHO required data elements, such as domestic violence, pain, and forms completion.
  • Assists and/or prepares patients for surgical procedures, tests, treatments and other diagnostic or therapeutic procedures.
  • Assists the health care provider in determining current and potential needs and /or problems with the patient
  • Document all patient assessment elements obtained.
  • Participates in the development, implementation, on-going evaluation and revision of nursing procedures
  • Assists in the evaluation of supplies, equipment, and facilities
