About The Position

Fairmount Homes is seeking a dedicated Licensed Practical Nurse (LPN) / Charge Nurse to join our Personal Care team. Located on a beautiful 70+ acre campus in rural Ephrata, Pennsylvania, Fairmount Homes is a five-star rated Continuing Care Retirement Community founded on Christ's love and Mennonite values. As a Charge Nurse in our Personal Care neighborhood, you'll play a vital role in supporting residents, guiding Resident Assistants, and helping create a warm, caring environment where residents feel safe, valued, and at home.
Requirements
- Current Pennsylvania Licensed Practical Nurse (LPN) License required
- Strong clinical judgment and decision-making skills
- Ability to lead and support a team in a positive and respectful manner
- Commitment to resident-centered care and service excellence
- Strong communication and organizational skills
- Ability to remain calm and professional during challenging situations

Responsibilities
- Provide clinical leadership and compassionate care while supporting the daily operations of the Personal Care neighborhood.
- Administering medications and treatments according to physician orders and facility policies
- Monitoring resident health and responding appropriately to changes in condition
- Making resident rounds to ensure safety, comfort, and quality care
- Completing required clinical documentation, progress notes, and resident records
- Supervising and supporting Resident Assistants while fostering accountability and teamwork
- Communicating resident needs and updates to team members and leadership
- Assisting with resident assessments, admissions, transfers, discharges, and end-of-life care
- Receiving and processing physician orders in accordance with facility policies
- Maintaining infection prevention, safety, and regulatory compliance standards
- Completing incident and accident reports as required
- Participating in team meetings and quality improvement efforts
- Supporting residents and families with compassion, professionalism, and respect
