About The Position

Angels of Care Home Health is seeking Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in the homes of medically fragile infants, children, and/or adolescents. The role involves focusing on the patient's medical needs and treatment according to the physician's plan of care, conducting ongoing assessments, administering prescribed medications and therapies, coordinating care, educating family members, and implementing preventative initiatives to ensure quality of care.

Requirements

• Active RN or LPN/LVN license (New Grads Welcome, training provided!)
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
