About The Position
This position involves supporting adults diagnosed with or affected by a disability in a case management setting, assisting them with the management of their healthcare. LifeScape focuses on empowering people to live their best life through a multi-disciplinary team approach. The RNs and LPNs are crucial in providing healthcare to individuals in the residential facility and those receiving day services. The role requires assessing the health status and wellbeing of individuals, analyzing clinical information, and managing conditions through assessment, planning, implementation, and evaluation.
Requirements
- Registered Nurse: A graduate of an accredited Registered Nurse (RN) program is required. A valid South Dakota RN Licensure required.
- Licensed Practical Nurse: A graduate of an accredited school of nursing is required. A valid South Dakota LPN Licensure is required.
- Any prior nursing and/or direct care experience is preferred.
- A valid driver's license with a clean record is required.
- All positions require a clear background check and drug screen upon conditional offer.

Responsibilities
- Provides respiratory care/support
- Interprets lab values and data
- Documents medical records electronically
- Educates/teaches staff, families, patients, students, and persons supported
- Promotes health and safety
