This Licensed Practical Nurse role functions as a patient navigator, guiding incoming calls and facilitating the entrance of new patients into the program. The Nurse Navigator provides patient education, promotes understanding of conditions, and ensures ongoing continuity of care. This role involves clinical support to providers and patients for diagnosis and treatment, operating in a team-oriented environment.

Requirements

Responsibilities

  • Perform duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program.
  • Provide patient education, promote an understanding of the condition, and provide ongoing continuity of care for patients.
  • Be accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena.
  • Assist other team members on the unit.
  • Provide clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
  • Utilize clinical knowledge to determine actions necessary to facilitate patient introduction to the program.
  • Perform and document evaluation of patients needs primarily by telephone or review of documentation.
  • Effectively collaborate with other healthcare providers in coordinating and implementing the patients plan of care, to include evaluation, treatment, and surveillance.
  • Effectively communicate with patient via mail, telephone, and in person regarding information pertinent to their care.
  • Assure pertinent patient information is received and available for provider interaction with patient.
  • Coordinate patient needs for the program.
  • Serve as the point of contact for providers.
  • Coordinate patient activities, such as support groups, educational series, as appropriate.
  • Provide limited patient and family education based on individual needs either in person or by telephone.
  • Coordinate patient activities through available support services, as appropriate.
  • Maintain systems and processes sufficient to assure timely follow-up and surveillance of patients for both Programs.
  • Serve as a liaison with Scheduling Services to ensure appropriate scheduling of patients.
  • Develop and implement patient screening tools and efficient clinic procedures.
  • Assist in the development and implementation of efficient patient navigational procedures.
  • Investigate patient concerns including appointment access, communication, and billing.
  • Maintain clinical supply inventory.
  • Be responsible for conducting medical record reviews to report findings to physician specialist.
  • Provide nursing care and assist physician and procedures.
  • Routinely inspect and observe condition and operation of equipment to ensure proper operation and identify the need for preventive and corrective maintenance per guideline.
  • Perform ongoing maintenance of exam and treatment rooms.
