About The Position

This position assists the Director of Nursing in planning, developing, organizing, and coordinating the day-to-day functions of the unit. The LPN NOC SHIFT is responsible for the overall coordination of the Care Center, including supervising Med Nurses and NAC documentation, resident assessments, care plans, admissions and discharges, and coordinating physician Medicare Part A certifications and appointments. This role also involves leading various meetings such as Triple checks, PDPM, and QA meetings, and ensuring functional maintenance programs for residents on the unit are managed. The position requires adherence to current RAI Manual guidelines for assessment coding, scheduling of assessments, and timely transmission of data. The LPN NOC SHIFT will also facilitate care plan updates, maintain care conference schedules, and act as a liaison between families and the Home. Additionally, the role involves treating residents with dignity and respect, promoting resident rights, establishing a culture of compliance, and following safety and hygiene measures. Confidentiality of protected health information is paramount, and any noncompliance, retaliation, discrimination, or allegations of abuse must be reported. The position also requires participation in life safety and emergency drills, reporting work-related injuries, and adhering to infection control policies. As a condition of employment, all assigned training and skills competency must be completed, including assigned portions of MDS and attendance at care-plan meetings. The LPN NOC SHIFT may accept licensed nurse assignments and perform licensed nurse duties as assigned, using proper lifting and body mechanics. The role requires the ability to work beyond normal working hours and on weekends and holidays when necessary, and to assist in evacuation of residents during emergency situations. The position may involve exposure to various hazards and emotional residents/family members. Leadership and supervisory abilities are essential, as is the ability to work independently and manage time effectively. Strong communication, listening, and interpersonal skills are required, along with basic computer proficiency in electronic medical record systems. The role may be subject to call back for emergency situations.

  • A nursing degree from an accredited college or university.
  • Minimum two (2) years of experience providing direct care in long-term care, restorative or geriatric setting as an LPN/RN.
  • Minimum (1) year of experience in nursing management.
  • Current CPR Certification.
  • Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against professional license.

Responsibilities

  • Keep the Director of Nursing Services informed regarding problems, concerns, and needs within the department.
  • Work as a team with other facility departments.
  • Responsible for the overall coordination of the Care Center, including: Supervise Med Nurses and NAC documentation, Resident Assessments, Care Plans including the Functional Maintenance Section, Overseeing/completing admissions and discharges, Coordinate physician Medicare Part A certifications and appointments.
  • Leading Triple checks weekly, PDPM meetings, and QA meetings.
  • Functional maintenance programs for residents on the unit.
  • May have to cover for Charge nurses and team leaders when needed.
  • Utilizes the current RAI Manual as a resource during the assessment coding process.
  • Schedules the Assessment Reference Dates (ARDs) for payment assessments in a manner that accurately captures each resident’s clinical characteristics for payment classification purposes.
  • Schedules the ARDs for all OBRA (Omnibus Budget Reconciliation Act) assessments daily, monthly, and as needed.
  • Provides a schedule of ARDs and assessment types weekly and as needed to the interdisciplinary team (IDT) in order to facilitate the timely completion of MDS sections and CAAs (Care Area Assessments) by each discipline.
  • Communicates with members of the IDT as needed for timely completion of assessments.
  • Expected to report any issues with timeliness to the Administrator and DNS/ADON.
  • Transmits assessments in accordance with current regulations.
  • To facilitate timely receipt of validation reports, will transmit as frequently(daily) as necessary to obtain timely validation of MD acceptance into the Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP) System data base.
  • Confirms transmission files by review and printing of initial and final validation reports.
  • Corrects any fatal errors immediately and retransmits the assessment until an accepted validation report is received.
  • Addresses non-fatal errors using the QIES ASAP System MDS 3.0 Provider User’s Guide.
  • Ensures the Business Office designee receives the initial and final validation report that acknowledges the acceptance of the MDS into the QIES ASAP.
  • Facilitates the completion and updating of resident care plans with the IDT to reflect each resident’s current needs.
  • Communicates with the IDT to identify residents in need of a Significant Change in Status assessment, based on criteria in the current RAI manual.
  • Participates in all resource utilization and/or triple check meetings prior to billing of claims associated with payment assessments.
  • Assists with admission, discharge, or care of residents as needed.
  • Attends or participates in continuing education related to the RAI process and related programs.
  • Completes all assigned training and skills competency, as determined by the facility assessment and facility training plans.
  • Performs administrative duties as assigned, including but not limited to QAPI activities and participation in department or committee meetings.
  • Initiate Care Plan updates on an ongoing basis to ensure continuous accuracy.
  • Maintain Care Conference schedule and coordinate documentation flow.
  • Obtain resident signatures on necessary documents.
  • Obtaining MOU when appropriate.
  • Obtain signatures from appropriate DSHS caseworkers.
  • Keeps abreast of government and Home requirements.
  • Implements and enforces these requirements.
  • Must be able to lead and work as a member of the health care team.
  • Must have basic computer skills, including data entry and word processing.
  • Acts as a liaison between the family members and the Home.
  • Other duties as necessary or assigned.

  • Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator.
  • Protects residents from abuse, and cooperates with all investigations.
  • Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
  • Participates in all life safety and emergency drills and trainings.
  • Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan.
  • Reports work-related injuries and illnesses immediately to supervisor.
  • Follows established infection control policies and procedures.
  • As a condition of employment, completes all assigned training and skills competency.
  • Completes assigned portions of MDS.
  • Attends care-plan meetings of assigned residents.
  • Participates in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.
  • Accepts licensed nurse assignments, as staffing needs require.
  • Performs licensed nurse duties as assigned, in accordance with facility policies and procedures.
  • Uses proper lifting and body mechanics while delivering care to residents.
