Requirements

  • LPN must be licensed to practice in the state of Georgia
  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly
  • Must possess basic computer and typing skills
  • Ability to work in Adult Outpatient

Responsibilities

  • Establishes individualized patient care by assessing patient needs, developing care goals, monitoring progress, and evaluating outcomes to ensure high-quality, patient-centered care.
  • Provides compassionate emotional, psychological, and spiritual support to patients and their families while fostering a respectful and supportive care environment.
  • Educates patients and families regarding medical conditions, medications, treatment plans, and self-care skills; promotes patient independence and answers questions regarding care.
  • Collaborates with physicians, physician assistants, nurse practitioners, and multidisciplinary healthcare teams both in clinic and through telehealth to coordinate comprehensive patient care.
  • Administers medications and injections safely, monitors patients for adverse reactions or side effects, and follows medication administration, storage, and controlled substance regulations.
  • Performs phlebotomy and obtains, documents, and monitors vital signs and other pertinent medical and psychiatric information.
  • Observes, documents, and communicates changes in patient conditions, symptoms, treatment responses, and health outcomes to appropriate healthcare providers.
  • Maintains complete, accurate, and timely medical records, reports, and clinical documentation while adhering to HIPAA and patient confidentiality requirements.
  • Completes prior authorizations and patient assistance applications accurately and efficiently to support timely access to prescribed treatments and services.
  • Responds to patient phone calls, messages, and questions in a timely and professional manner, providing appropriate information and escalating concerns when necessary.
  • Maintains a safe, clean, and organized clinical environment by following infection-control policies, safety procedures, hospital protocols, and applicable nursing standards and regulations.
  • Addresses patient needs and concerns through effective communication, collaboration, and multidisciplinary problem-solving while utilizing healthcare support personnel when appropriate.
  • Manages multiple clinical and administrative responsibilities effectively while working autonomously and collaboratively to maintain a high standard of patient care.
