About The Position

Avita Health System is seeking a dedicated Licensed Practical Nurse (LPN) to join their MedSurg Department at the Ontario location. The LPN will gather data, plan, and implement nursing care for patients under the supervision of an RN, ensuring continuity of care between shifts. This role involves providing direct patient care, assisting nursing assistants, and participating in patient and family teaching, all while acting as a patient advocate aligned with the facility's mission.

Requirements

Current licensure as a Licensed Practical Nurse in State of Ohio.
Current BLS certification.

Nice To Haves

Two (2) or more years of previous experience in rehabilitation nursing, longer term care or hospital setting.

Responsibilities

Gathers data, plans and implements the nursing care of patients within the nursing unit under the supervision of a RN.
Ensures continuity of care of the patients between shifts by providing direct care as well as assisting with the care given by nursing assistants and supportive staff members.
Participates in patient and family teaching.
Maintains role as patient advocate with a focus on the facility’s mission.
