About The Position

Wyandot BHN is seeking a dedicated Licensed Practical Nurse or Registered Nurse to join our Med Clinic! The purpose of the LPN or RN position is to provide support to adult clients who receive care from the medication clinic by providing professional nursing care, care management and coordination services.

Requirements

  • Must be licensed in Kansas by the Kansas Board of Nursing as a Licensed Practical Nurse.
  • Familiarity with mental health theory, emerging promising practices and Evidence-Based practices.
  • Knowledge of medical and non-medical services delivery system.
  • Effective communication with clients, family and supports, guardians and other services providers.
  • Ability to establish relationships with individuals and organizations of influence within the social services community.
  • Demonstrated strong interpersonal and communication skills.

Nice To Haves

  • Prior experience with mental health is preferred.

Responsibilities

  • Engage individuals and families as partners in services and develop positive, hopeful and trusting relationships.
  • Assists individuals and families in avoiding and resolving crisis situations.
  • Works closely to provide education and support to clients, families, or other support persons the client identifies.
  • Provides care coordination with other treatment providers, both internally and externally.
  • Supports the prescribing physician and/or nurse practitioners as needed.
  • Provides assistance as needed for scheduling within the medication clinic.
  • Works with the Nurse Manager and administrative assistant to maintain an inventory of medical and office supplies for the medication clinic.
  • Interacts with pharmaceutical representatives as needed to coordinate with other providers.
  • Assists with making referrals for other services within Wyandot Center and other medical and psychiatric facilities in the community according to the prescriber’s order.
  • Works with staff to prioritize urgent needs.
  • Returns phone calls that come to the clinic services to determine level of need and develop a plan for accessing care.
  • Works with the pharmacy to coordinate prescription assistance plans.
  • Administer medications to clients as needed including but not limited to administering injections.
  • Thoroughly and timely completes all documentation and reporting requirements.
  • Demonstrates positive interactions with clients, other members of the agency and external community partners.
  • Is familiar with and complies with all relevant policies and procedures.
  • Performs other duties as deemed appropriate by immediate supervisor or designee.
