About The Position

Carebridge Health, a member of the Elevance Health family of companies within Carelon business, aims to maximize individuals' health, independence, and quality of life through home and community-based services. This is a 100% virtual role, offering maximum flexibility and autonomy, with the exception of required in-person training sessions. Candidates must be within a reasonable commuting distance from the posting location(s) unless an accommodation is granted. The LPN/LVN will examine and treat patients under the direction of a physician, review medical records, interview patients, record vital signs, provide nursing interventions and coaching, facilitate patient transfers, and document care using standard templates. The role requires the ability to be on the phone engaging with patients up to 80% of the shift and working assigned weekends per standard team rotation, along with reliable internet and strong customer service skills.

Requirements

Requires an LPN or LVN and minimum of 2 years of experience as an LPN/LVN; or any combination of education and experience, which would provide an equivalent background.
Current LPN/LVN license in the applicable state required.
Satisfactory completion of a Tuberculosis test is a requirement for this position.

Nice To Haves

Bi-lingual (English/Spanish) preferred.
Experience with Telephonic Triage preferred.
Acute care experience (e.g., urgent care or ER) preferred.

Responsibilities

Reviews patient medical records, interviews patients and records vital signs.
Provides nursing interventions and coaching in accordance with the scope of practice and standing orders.
Facilitates patient transfers to alternate level of care.
Documents using standard templates.
Ability to be on the phone engaging with patients up to 80% of the shift.
Ability to work assigned weekends per standard team rotation.
