About The Position

Tryfacta is seeking a LPN /LVN for our client in Rochester, NY 14626-4164. This is a 13-week contract assignment. First Time Travelers are Accepted. Requirements

• 1+ Year of Acute Care Experience is Required
• BLS
• LPN License

• Epic EMR is Preferred

Responsibilities

• Provide direct nursing care to Med-Surg/Telemetry patients under RN supervision and according to physician orders.
• Monitor vital signs, cardiac rhythms, oxygen saturation, pain levels, and changes in patient condition.
• Administer medications and treatments, including oral, subcutaneous, and IV medications within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other prescribed nursing procedures.
• Monitor telemetry patients for changes in cardiac rhythm and promptly report abnormal findings.
• Assist with admissions, transfers, discharges, and activities of daily living (ADLs).
• Maintain accurate and timely documentation of assessments, medications, treatments, and patient responses in the EMR.
• Collaborate with RNs, physicians, therapists, and other interdisciplinary team members to ensure safe, quality patient care.
