About The Position
RPM Healthcare is seeking a licensed LPN/LVN to serve as an RPM 365 Care Coach. This role involves engaging patients, tracking and analyzing their physiological data (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation), identifying dangerous trends, providing education, and encouraging adherence to care plans. The ideal candidate is passionate about healthcare innovation, tech-savvy, comfortable with high-volume phone outreach, and thrives in a fast-changing environment. They should be patient, empathetic, detail-oriented, and a self-starter.
Requirements
- Active New York State nursing license.
- Knowledge of chronic conditions and related monitoring devices.
- Proficiency with technology, computer platforms, and smartphones.
- Strong verbal/written communication skills.
- Comfortable with patient outreach and engagement.
- Highly organized, detail-oriented, and team-focused.
- Independently motivated.
- Comfortable spending the majority of your workday engaging with patients over the phone providing support, education and guidance.
Nice To Haves
- Experience in telehealth, remote patient monitoring, education, or customer service.
- Experience in weight management coaching.
- Additional language skills are a plus.
- RPM experience a plus.
Responsibilities
- Serve as the primary contact for patients on the RPM365 platform.
- Assist with device setup and app connectivity.
- Engage with patients to ensure adherence to care plans.
- Schedule monthly (or more) check-ins and monitor patient data.
- Educate patients through shared resources.
- Notify healthcare providers of critical alerts.
- Attend virtual team meetings for updates.
