About The Position

Sage Group HealthCare Staffing is seeking LTC LPN/LVN for a travel/contract position in Claremont, NH. This is a full-time, 13-week contract role in a Long-Term Care unit. The position requires a minimum of 3 years of recent LVN/LPN experience, specifically in Long-Term Care. The role involves observing residents, administering medications and treatments, documenting care, and supervising LNAs. The traveler must have their own reliable transportation as ride-sharing or public transport is not available at this location. Covid Vaccination Series and Booster are required.

Requirements

Reside beyond a 50-mile radius of the facility location.
3 years of recent/current LVN/LPN experience (recent within 6 months).
LTC experience.
Must be willing to float.
Covid Vaccination Series and Booster Required.
LPN/LVN License for State of NH or compact.
BLS Certifications from AHA.
Traveler must have their own reliable transportation.

Nice To Haves

IV Certification.
Rural/small facility experience.
Prior travel or contract assignment experience.

Responsibilities

Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician; and receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
Accurately receive, transcribe, and carry out physician’s orders.
Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
Order and maintain all pharmaceuticals for the units.
Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing.
Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given; and assist direct-care staff with nursing care as necessary and as time allows.
