About The Position
Sage Group HealthCare Staffing is seeking LTC LPN/LVN for a travel/contract position in Claremont, NH. This is a full-time, 13-week contract assignment. The position involves observing residents, administering medications and treatments, transcribing physician orders, and supervising LNAs. The role requires a minimum of 3 years of recent LPN/LVN experience, specifically in Long-Term Care. Candidates must be willing to float and have their own reliable transportation, as ride-sharing or public transport is not available at this location. COVID-19 vaccination series and booster are required.
Requirements
- Reside beyond a 50-mile radius of the facility location.
- Recent/Current LVN/LPN experience (recent within 6 months): 3 years
- LTC experience
- Must be willing to float
- LPN/LVN License for State of NH or compact
- BLS Certifications from AHA
- Traveler must have their own reliable transportation
- Covid Vaccination Series and Booster Required

- IV Certification
- Rural/small facility experience
- Prior travel or contract assignment experience
Responsibilities
- Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician.
- Receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
- Accurately receive, transcribe, and carry out physician’s orders.
- Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
- Order and maintain all pharmaceuticals for the units.
- Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing.
- Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given.
- Assist direct-care staff with nursing care as necessary and as time allows.
