About The Position
Navitas Healthcare, LLC is seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide care for individuals with developmental disabilities in a long-term care setting. The LPN will work under the supervision of a Registered Nurse and assist with medication administration, treatments, patient monitoring, and daily care activities.
Requirements
- Active New York State LPN License
- Minimum one year of nursing experience after licensure
- SCR and SEL clearance required prior to start (may take 2–4 weeks)
- Strong communication and patient care skills
- Ability to work independently and as part of a healthcare team
Nice To Haves
- Experience in LTC, developmental disabilities, or residential care preferred
- Venipuncture certification preferred
Responsibilities
- Administer medications and injections as prescribed
- Perform tube feedings and hang IVs as required
- Collect blood samples (venipuncture certification preferred)
- Provide colostomy care and wound care
- Monitor patient conditions and report changes to nursing leadership
- Take and record vital signs including temperature, pulse, respiration, and blood pressure
- Collect specimens for laboratory analysis
- Apply dressings and perform basic nursing procedures
- Assist with bathing, feeding, lifting, and transferring residents
- Maintain resident living areas and assist with daily care needs
- Complete accurate documentation and patient records
- Float to other homes/facilities as needed based on staffing requirements
