About The Position

Seeking experienced Licensed Practical Nurses (LPNs) for a Long Term Care and Skilled Nursing environment caring for residents with complex medical needs. The role involves providing direct nursing care, administering medications and treatments, monitoring resident conditions, and collaborating with healthcare teams. The facility is a Long Term Care / Nursing Home / Skilled Unit / LTAC, utilizing the Matrix Charting System. The nurse-to-patient ratio on nights is approximately 1:24 – 1:28.

Requirements

• Active LPN License required
• Ability to work rotating night schedules
• Must be willing to work every other weekend
• Strong clinical and communication skills
• Reliable attendance and professional work ethic

Nice To Haves

• Previous LTC or Skilled Nursing experience preferred
• Comfortable caring for trach, vent, wound care, and tube feed patients
• IV Certification preferred
• LTAC experience preferred
• Matrix Charting experience preferred
• BLS Certification preferred

Responsibilities

• Provide direct nursing care to LTC and skilled nursing residents
• Administer medications and treatments as ordered
• Monitor and document resident conditions
• Provide trach and vent care
• Perform wound care and tube feed management
• Maintain accurate nursing documentation
• Collaborate with physicians and interdisciplinary teams
• Follow infection control and patient safety protocols
• Support resident comfort and quality of care
• Respond appropriately to changes in patient condition
