LPN Long Term Care (LTC)
About The Position
The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role demonstrates knowledge of nursing principles, competency within the full scope of nursing expertise, and utilizes appropriate age and population specific standards. The LPN performs testing to evaluate resident status, recognizes problems, recommends solutions, and responds effectively in emergent situations. They ensure the health, comfort, and safety of residents, contribute to resident assessments, and administer medications or treatments as ordered. The position also involves providing technical support, documenting resident interactions and outcomes, and establishing effective working relationships within an interdisciplinary team. Additionally, the LPN counsels residents and families on prevention and treatment plans and develops self-care programs. Other related duties may be assigned.
Requirements
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department specific competencies and certifications.
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.
Nice To Haves
- Associate degree in practical nursing is preferred.
- If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
Responsibilities

- Performs testing to evaluate data reflective of the resident's status and provides necessary care.
- Recognizes problems, recommends solutions, and responds effectively to assist in emergent situations.
- Ensures the health, comfort, and safety of residents.
- Contributes to the assessment of residents.
- Administers medications or treatments as ordered.
- Provides technical support to healthcare professionals.
- Documents resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary.
- Establishes and maintains effective working relationships with residents, health care providers, and the public.
- Works effectively with professional and supportive personnel as a member of an interdisciplinary care team.
- Communicates clearly and maintains confidentiality of sensitive information.
- Counsels residents, family members, and/or resident advocates on prevention and treatment plans.
- Develops a self-care program using resident education principles.
- Performs other related duties as assigned.
