About The Position

Life Care Center of Evergreen is seeking a qualified Licensed Practical Nurse (LPN) to join their team. This facility, located just west of Lakewood, is a 5-star rated facility by CMS and has a 4.7 Google Star Rating. They are recognized for excellent customer service and have been identified by US News and Review for Best Nursing Home for 2026. The center's mission is to act with integrity towards residents, associates, and themselves, supporting associates who wish to serve seniors in the mountain community. They welcome new LPNs to the Long Term Care setting and offer support from their 'DreamTeam'. Life Care Center of Evergreen is affiliated with Life Care Centers of America, a privately owned industry leader in healthcare with over 200 locations. The LPN will be an important member of the interdisciplinary team, delivering quality nursing care to patients to help them achieve their highest practicable physical, mental, and psychosocial well-being, adhering to all applicable laws, regulations, and Life Care standards. The role includes monitoring and reporting changes in patient condition, and assigning duties, instructing, and providing clinical supervision to CNAs.

Requirements

  • Graduated from an accredited LPN program.
  • Currently licensed/registered in applicable State.
  • Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
  • CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
  • Make independent decisions when circumstances warrant such action.
  • Knowledgeably and competently deliver quality nursing care to patients.
  • Exhibit excellent customer service and a positive attitude towards patients.

Nice To Haves

  • One (1) year skilled nursing experience preferred.

Responsibilities

  • Deliver quality nursing care to patients through interpersonal contact.
  • Provide care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards.
  • Monitor and report changes in the patient’s condition.
  • Assign duties, instruct, and provide clinical supervision to CNAs.
