Provides nursing care under the supervision of an RN, for assigned patients in assigned areas practicing Evidence-Based nursing as the standard. This role involves providing nursing care appropriate to the age, needs, and abilities of the resident/patient, including medication and treatment administration, teaching, and implementation of the interdisciplinary care plan. The LPN contributes to care plan development by collecting and recording data, communicates and collaborates effectively with residents, families, colleagues, and the interdisciplinary team, and documents accurate, pertinent, and comprehensive information in a timely and concise manner using the Electronic Medical Record. Additionally, the LPN supervises the care provided by CNAs, participates in educational programs and quality improvement initiatives, and follows the standards of the Diligent Minimal Lift Program.
