LPN (Licensed Practical Nurse) Home Health
About The Position
As an LPN at Phoenix Home Health, you serve as a link between patients and their medical providers. You use physical assessment and communication skills to carry out care plans based on physician orders and initial assessments. You collaborate with nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Charting is completed on-site with assistance from Roger’s AI-powered support, rather than being taken home. The position serves Branson, Missouri, and surrounding areas. The listed schedule is Monday through Friday, 8 a.m. to 5 p.m., with a weekend rotation every four to six weeks.
Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing License.
- Strong organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver’s license.
Responsibilities
- Assist in reviewing and revising the plan of care.
- Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
- Observe and record the client’s reaction to treatment and report changes in the client’s condition to appropriate personnel and/or the physician.
- Provide ongoing instruction to the client or family about the plan of treatment.
- Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
- Collaborate with nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.
