Requirements

  • 18 years of older
  • Able to pass an Oregon background check
  • Minimum 1 year LPN experience in related field/setting
  • Possess and maintain strong knowledge of Oregon Board of Nursing requirements, especially as they pertain to Nursing Delegation
  • Possess and maintain a strong ability to teach and train on all components of Nurse Delegation
  • Possess a thorough knowledge of Med Room components and Medication Administration
  • Must possess an active Oregon-endorsed LPN license
  • Reliable Transportation
  • Must be a U.S. Citizen and reside within the State of Oregon to apply.
  • Have reliable means of transportation/commuting to work.

Responsibilities

  • Develop and support a system that allows for a constant flow of communication to the Director of Nursing regarding resident, family, or employee concerns, a decline in resident condition, employee injuries, budget disparities, or any matters that potentially impact operations
  • Review and perform evaluations of resident health and functional care needs before moving in to ensure that the community can meet the resident’s needs and that regulatory requirements have been met
  • Review all resident information and complete an assessment of the resident before move-in, when changes in the resident's condition occur, and as required by company policy and state regulation
  • Assume on-call responsibilities on a rotational basis and assign on-call responsibilities to other qualified management
  • Develop knowledge of community policies, procedures, and state/federal regulations pertaining to resident care
  • Perform selected clinical tasks that reflect an unusual or unique condition in a resident or that cannot be delegated, i.e. IM injection
  • Monitor the status of resident’s health condition and initiate actions in response to changing needs
  • Assist in coordinating resident transfers, to or from a hospital or another health facility
