About The Position

The LPN works under the supervision of leadership and the direction of the RN and/or Provider. The LPN administers quality patient care according to established standards and may be required to work in other areas according to skill of staff and acuity of patients. The role provides exceptional service to all patients and customers.

Requirements

  • Degree from an accredited school of practical nursing, or military equivalent.
  • Licensed Practical Nurse (LPN) - State Licensure - State Licensure/Or Compact State Licensure in state, depending upon designated work location - Required Upon Hire
  • IV Certification - IV certified upon hire - Required Upon Hire
  • Basic Life Support (BLS) - Required within 90 Days
  • Advanced Cardiac Life Support (ACLS) - (Albany Specific) - Required within 90 Days
  • Must be able to travel between various system facilities and off-site locations as needed.
  • Fine motor skills, eye/hand coordination, answer phone, type.
  • Ability to give injections, administers medications, and change dressings.
  • Displays a friendly, positive attitude and communicates with patients, families, physicians and co-workers in a professional manner.
  • Must walk, move, kneel, crouch, stoop, reach, stand, pull, push, and lift, speak clearly
  • Ability to collect thorough information regarding patient’s condition and evaluate effects of care.
  • Problem solve, manipulate numbers, adaptability, interpret data, calculate, comprehend, explain, assess, evaluate, short term and long term memory.
  • Ability to visually assess patient condition; must be able to communicate with patient in order to assess condition and evaluate effects of care.
  • Visual, hearing, feeling, smell.

Responsibilities

  • Completes daily reassessments for assigned patients and documents findings.
  • Updates and implements the plan of care.
  • Reinforces patient teaching and family education.
  • Administers medications and monitors patients’ responses to medications including intravenous fluids and medications.
  • Collaborates effectively with all members of the interdisciplinary team.
  • Contributes to the professional development of peers, colleagues, and others (students, orientees).
  • Other duties as assigned
