We are looking to hire an LPN to join our team. -Assuring competent, compassionate nursing care for specific patients and families. -Responsible and accountable for the overall nursing care provided in clinic during a patients visit, including primary care for People Living With HIV/AIDS and Infectious Disease specialty care. -Collaborates with other health care providers and attending physicians in providing optimal health outcomes for patients.

Requirements

Licensed Practical Nurse required
Licensed Practical Nurse [LPN - State License] - Generic - HR Only required
Basic knowledge of patient education: interviewing and listening skills, teaching and engagement techniques, and understanding of health-related conditions and information.
Ability to accurately perform vital signs and obtain pertinent health information, including but not limited to: temperature, pulse, respirations, blood pressure, height, and weight.
Ability to administer medication; routes of administration may include injections, intravenous, and oral medications.
Knowledge and ability to perform specific procedures and tests as appropriate to the clinical setting.
Ability to communicate in a professional, courteous, clear, and concise manner.

Nice To Haves

Infectious disease/HIV experience preferred.
Proficient in speaking other languages, Spanish and/or Portuguese is strongly preferred.
Ambulatory experience preferred.
Substance use disorder treatment experience a plus
Microsoft Office and Epic computer literacy preferred.
Basic Life Support [BLS Certification] - Data Conversion - Various Issuers preferred
Medical experience 0-1 year preferred

Responsibilities

Provide initial patient assessments including falls screening and vital signs
Perform phlebotomy, provide SQ/IM injections and place peripheral intravenous lines for IV infusions.
Perform PICC line dressing changes, PICC line removal, and line malfunction interventions including administration of Alteplase.
Serve as a resource and oversee the operational flow of the clinic in collaboration with other nursing staff and medical assistants.
Provide education and follow up for patients on specific disease processes or therapies.
Coordinate care for and monitor patients on injectable therapies including HIV treatment and prevention therapies.
Collaborate with Community Health Workers to outreach to and follow up with high-risk primary care patients.
Serve as a liaison between patients and other members of the treatment team.
Provide nursing services for the Traveler’s and Tropical Medicine Clinic when needed.
Participate in quality assurance activities.
Collaborates with other professionals and directs nonprofessional nursing personnel in maintaining recognized standards.
Participate in orientation of new permanent and temporary staff members.
Appropriately utilizes nursing resources.
Adheres to hospital, departmental, divisional and clinic policies and procedures.
Participates in daily operational activities necessary for safe patient/staff environment.
Utilizes resources efficiently to facilitate optimal patient care.
Maintains and updates clinical knowledge and skills based on current nursing and health care practices.
Adheres to requirements for mandatory continuing education and certification.
Identifies own learning needs and goals and develops a plan to meet them.
Maintains and develops professional practice through continuing educational programs and appropriate learning experiences.
Contributes to relevant committees, professional meetings, continuing educational and/or formal academic programs.
Interviews patients about current problems, medications, and allergies, and reviews medical records for pertinent previous and current information.
Measure and record patients' vital signs. Based on the chief complaint, the physician may also perform additional screening exams as needed, such as hearing, vision, depression, and falls assessments.
Measure and record patients' vital signs, including height, weight, temperature, blood pressure, pulse, and respiration.
Provide basic patient care and treatments, such as dressing wounds, treating bedsores, administering enemas or douches, applying alcohol rubs, massaging, performing catheterizations, administering routine immunizations, and/or providing nebulizer treatments and visual acuity tests.
Participates in patient treatment regimen, including administering medications (under the supervision of a physician and registered nurse).
Communicates post-visit findings and participates in the plan of care, in coordination with the appropriate clinical staff
Performs a variety of office responsibilities: answers phones and takes messages, and performs triage within the scope of licensed practical nursing practice in an accurate and timely manner.
Collect samples, such as blood, urine, or sputum from patients, and perform routine laboratory tests on samples.
Prepare patients for examinations, tests, or treatments and explain procedures.
Assist patients with bathing, dressing, maintaining personal hygiene, transferring in bed, and standing and walking.
