LPN Hospice Intake and Triage
About The Position
Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. As a continuing care LPN with Corewell Health, providing top-notch patient care is paramount. Under the direction of an RN, mid-level Provider, or Physician, you'll be responsible for assessing, planning, implementing, and evaluating patient care. You'll be a vital part of our team, making a real difference in the lives of our patients and their families.
Requirements
- High School Diploma or equivalent
- Completion of an approved nursing program
- Successful completion of competency based orientation program
- License Practical Nursing (LPN) - State of Michigan Upon Hire
- Driver's License - State of Michigan Upon Hire
Nice To Haves
- Associate's Degree or equivalent
- Graduate of an approved nursing program
- Healthcare experience
