About The Position

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide emergent field visits throughout Palm Beach County for hospice patients & families in need of immediate support. This role is ideal for a nurse who thrives in independent practice, stays calm under pressure, and is driven by a sense of purpose, delivering skilled, responsive, in‑home care during the following hours: 6:00am - 4:30pm and every other weekend. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

Requirements

Current license to practice as a Licensed Practical Nurse in the State of Florida.
Current Basic Life Support Certification.
Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements.
Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.

Hospice Certification preferred.

Responsibilities

Supports and carries out the organization’s mission, vision, and values on a consistent basis.
Working under the direction of the registered nurse and primary physician, the Licensed Practical Nurse (LPN) is responsible for identifying patient/family needs, and coordinating the care for the patient/family.
The LPN will utilize monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
Pledge to make the comfort and care of patients and families the highest priority.
Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
Ensure the complete satisfaction of the patient and family.
