We are seeking a compassionate and motivated Licensed Practical Nurse LPN to join our Home Health team.
Requirements
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current Indiana LPN license
- Minimum of 1 year of nursing experience in a clinical care setting
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability and willingness to travel within assigned territory
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Nice To Haves
- Home Health or Hospice experience preferred
Responsibilities
- Provide skilled nursing care in accordance with physician orders and patient care plans
- Review and follow the Plan of Care during each patient visit
- Monitor patient conditions and communicate changes with the RN Case Manager and care team
- Educate patients and caregivers on disease management and self-care techniques
- Document patient care accurately and timely
- Collaborate with physicians, clinicians, and interdisciplinary care teams
- Support efforts to reduce avoidable hospitalizations and improve patient outcomes
- Deliver compassionate care and exceptional patient experience
