Requirements

• Active LPN license
• Home Health experience required
• 1+ year clinical experience
• Ability to work independently
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds

Responsibilities

• Skilled nursing visits in the home
• Follow Plan of Care
• Monitor and report patient changes
• Educate patients and caregivers
• Document visits accurately
