About The Position

Sage Group HealthCare Staffing is seeking LPN Home Health for multiple 13-week travel/contract positions in Zanesville, OH. This role involves providing and coordinating care for assigned Home Health patients under the supervision of an RN. The LPN will develop and implement nursing care plans, educate patients and families on disease management, and provide Home Health nursing care within the defined scope of practice. Responsibilities include ensuring physicians' orders are transcribed correctly, assessing and documenting patient status, and notifying physicians of significant changes. The territory for this position covers Morgan and Perry counties.

Requirements

LPN License for the state applying or Compact if accepted - Required
BLS Certification from AHA or American Red Cross - Required
LPN experience: 1 - 2 years - Required
Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
Home Care Home Base (HCHB) charting experience – Required

Responsibilities

Provide and coordinate care for assigned Home Health patients under supervision of RN
Develop and implement nursing care plans for patients as assigned.
Empower patients and families to reach healthcare goals by educating them on disease management.
Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
Notify physicians of significant changes in status.
