This role involves providing essential patient care in a home health setting, focusing on maintaining patient well-being and independence. The LPN will collaborate with healthcare teams, manage patient records, and ensure adherence to safety and infection control protocols. Specific duties include developing and implementing care plans, managing pain and symptoms, educating patients and families, and providing continuous care when appropriate. The position also requires understanding and compliance with home care requirements, including patient homebound status, and reporting patient status to the RN Case Manager. The role demands cognitive ability to function at a high level, maintain concentration, and ensure thoroughness and accuracy in all tasks. The Hospice LPN will function as a support nurse and core member of the Interdisciplinary Group, assisting with the development of the plan of care that addresses the physical, psychosocial, emotional, and spiritual needs of the terminally ill patient and their family. They will follow established guidelines for pain and symptom management to enhance quality of life and educate patients/families regarding the care needs at the end-of-life, including palliative care measures. Under the direction of the RN, the Hospice LPN will provide Continuous care services when appropriate.
